Eighty percent of human beings experiencing low back pain at some time during their lifetimes. This statement extrapolated from industrial statistics, medical records, and insurance figures. This percentage probably does not fully reflect the greater number of persons who have backaches that do not prevent them from working or performing activities of daily living but make their daily activities painful.
There are other disease of the body that can and do cause low back pain. These conditions include disease of kidney, stomach, pancreas, and bowel disorders. Malignancies, bone disease, metabolic and systemic disease could attributes to back pain. These disorders are always considered by the physicians especially after unusual back pain persistent, or fails to respond to the recommended standard treatments.
However, the most common known cause to Low back impairment is mechanical.
The normal function of the spine must be understood in order to understand abnormal function, which may cause pain and disability.
Somewhere in the tissues of the spine is a site, a spot, or a portion of those tissues that is or has been irritated, stressed, abused, injured, used improperly, deteriorated, or even diseased. Pain can result from this tissue insult.
The disc fibers of the annulus can stretch to a limited degree; therefore when the vertebrae are compressed together, the fibers stretch but do not tear. When the vertebrae bend on each other, the fibers can stretch enough to allow bending but do not tear. When the fibers are twisted, they are too limited to share and they may be stretched more than they can allow and thus tear. The outer fibers called annular fibrosus tear first and that what is appeared in MRI as bulge. When the disc be further damaged the tear keep moving toward the center which hold the small jelly substance called nucleus pulpous, if it busted now the disc become herniated. When more pressure was applied, nucleus material keep finding ways out the annular pulpous, so it bleed through the annular crack until small portion be able to go out and lie on the nerve root and now this called extrusion. This and earlier factors produce narrow in nerve gutter that may lead to neuro-ischemia that become a source of pain in the muscle that innervated by the anterior and posterior rami.
The ligaments and the facets can attribute great deal to the back pain.
Remedies that we must consider:
Pain can be evaluated, understood, and remedied if the injury and the specific tissue site are clarified.
Lumbar lordosis must be rebuilt. This is the key for retuning the spinal column to its resting physiological position, so annular and nucleus can be protected. The passive (ligaments) and active (muscles) structures can be relaxed and healed.
Passive structure must be strengthen, ability to support without be torn during motion is critical.
Improve muscle strength:
Muscle weakness can lead to not only repetitive sprain but also produces skeletal deformities that could be hard to correct. The muscle power is the best to brace the body and cage it against deterioration. Muscles provide heat and accommodate stress.
Patient must learn how to use their back, e.g. now we know that twisting is bad for low back healthy structures. Patient must learn how to lift, to sit, to stand, to turn, to read, etc.
